Identification of purchaser

Ark. Code Ann. § 5-64-1002

(a) Any manufacturer, wholesaler, retailer, or other person required to maintain records of transactions under this subchapter shall obtain proper identification from the purchaser.

(b) “Proper identification” means a motor vehicle operator's license or other official state-issued identification of the purchaser that contains a photograph of the purchaser, and includes:(1) The residential or mailing address of the purchaser, other than a post office box number;(2) The motor vehicle license number of any motor vehicle owned or operated by the purchaser; and(3) A letter of authorization from the business for which any substance specified in § 5-64-1001 is being furnished, that includes:(A) The business license number and address of the business;(B) A full description of how the substance is to be used; and(C) The signature of the purchaser.

(3) A letter of authorization from the business for which any substance specified in § 5-64-1001 is being furnished, that includes:(A) The business license number and address of the business;(B) A full description of how the substance is to be used; and(C) The signature of the purchaser.

(c) The person selling, transferring, or otherwise furnishing any substance specified in § 5-64-1001 shall affix his or her signature as a witness to the signature and identification of the purchaser.
