(1) A petition shall set forth the following:(1) The name, address, and if known, the date of birth of the maltreated adult who shall be designated as the respondent;(2) The maltreated adult's current location;(3) The name and address of the maltreated adult's closest adult relative, if known;(4) (A) The facts intended to prove the person to be maltreated.(B) The facts may be set out in an affidavit attached to the petition and incorporated into the petition; and(5) The relief requested by the petitioner.
