Every prepaid limited health service organization subject to this act must pay to the office the following fees:(1) For filing an application for a certificate of authority or amendment thereto: $500.(2) For filing each annual report: $200.(3) For each renewal of certificate of authority: $500.History.—s. 46, ch. 93-148; s. 1548, ch. 2003-261.
