No later than the fifteenth (15th) day of each month, the Director of Public Health and Social Services shall furnish the Commission with an abstract of the register of deaths for all decedents sixteen (16) years of age and over, showing the: (a) full name; (b) sex; (c) date of birth; (d) nativity/place of birth; (e) home address; and (f) month, day and year of death.
