54-1733A. PRESCRIPTION DRUG ORDER MINIMUM REQUIREMENTS. (1) A prescription drug order may be transmitted by delivery of the original signed written prescription or a digital image of the order, or by a prescriber, prescriber’s agent, or representative of a state-licensed or federally certified provider community either electronically, verbally, or via facsimile. (2) Each prescription drug order shall include at least the following: (a) The name of the patient or authorized entity, and, if for an animal, the species; (b) If a controlled substance, the patient’s address; (c) The date issued; (d) The drug name, strength, and quantity; (e) Directions for use; (f) The name of the prescriber, and, if a controlled substance, the address and DEA registration number; and (g) The signature of the prescriber or, if a renewal of a previous prescription, the prescriber’s authorized agent. (3) A provider may omit drug information and directions and, instead, make an indication for the pharmacist to finalize the patient’s drug therapy plan.
