54-5808. APPLICATIONS. Each applicant for a license, certificate or registration shall: (1) Make application to the board on forms authorized and furnished by the board, such application to contain proof under oath by the applicant of the particular qualifications of the applicant; (2) Pay to the board the required fee; and (3) Provide documentation and information to establish that the applicant meets the requirements for the license, certificate or registration sought.
