Every order issued by the corporate authorities of a district as described in Section 32-4.14 shall state for what purpose and on what account it is issued, and shall be in the following form: STATE OF ILLINOIS $....... No. ....

Pay to the order of .... the sum of .... Dollars ($....) for .....
