Resources and sample forms

Sec. 30. (a) The state department shall maintain a list of resources on its Internet web site, including sample advance directive forms that are consistent with this chapter. (b) A declarant is not required to use any official or unofficial form to prepare and sign a valid advance directive. As added by P.L.50-2021, SEC.63.
