§2415. Issuance of license; updating contact information A. If an applicant meets the requirements of this Chapter, the board shall issue the applicant a license to practice physical therapy. B. Each licensee is responsible for reporting a name change and changes in business and home address, telephone numbers, and e-mail address to the board within thirty days of such change.
