§ 30302. Resident Identification Cards. (a) Upon payment of the required fee and determination by the Office of the Mayor of lawful residents status, the Office of the Mayor shall issue to a person, a resident identification card. The application shall be accompanied by a certificate copy of the applicant’s birth certificate, a passport, U.S. social security number or any other document determined to be necessary by the Office of the Mayor for the expeditious processing of the identification card. (b) At a minimum, the card shall have on it a distinguishing number assigned to the card, the full name, sex, date of birth, residence address, a brief description including a photograph, if possible, of the cardholder and either a facsimile of the signature of the cardholder or a space upon which the cardholder shall write his usual signature with pen and ink immediately upon receipt of the identification card.
