Mobile dental facilities - information to patients

Except as provided in section 4715.73 of the Revised Code, a mobile dental facility operator or the operator's representative shall provide each patient receiving dental or dental hygiene services at a mobile dental facility, or the patient's representative, with all of the following: (A) The name of each individual who arranged for or provided dental or dental hygiene services to the patient; (B) The telephone number to reach the mobile dental facility operator or operator's representative in case of an emergency; (C) A list of dental or dental hygiene services provided to the patient; (D) Any recommendations regarding further dental or dental hygiene services that are advisable; (E) A notice to the patient that the facility must provide access to the patient's complete dental records in accordance with Chapter 3798. of the Revised Code and applicable federal laws; (F) Instructions for requesting a copy or transfer of the patient's records.
