Acceptance and use of form

A. A health care entity shall accept the authorization form adopted and distributed by the State Board of Health as a valid authorization for the exchange of health information. B. A health care entity is not required under this act to use the authorization form adopted and distributed by the Board. Added by Laws 2008, c. 305, § 5, emerg. eff. June 2, 2008.
