Reissuance of License

Sec. 201.511. REISSUANCE OF LICENSE. (a) On application, the board may reissue a license to practice chiropractic to a person whose license has been canceled or suspended. (b) An applicant whose license has been canceled or revoked: (1) may not apply for reissuance before the first anniversary of the date the license was canceled or revoked; and (2) must apply for reissuance in the manner and form required by the board.
