Form of Notice of Substitution

Sec. 274.104. FORM OF NOTICE OF SUBSTITUTION. The notice required under Section 274.103 must be in writing and disclose: (1) the effect the substitution of fiduciary will have on the situs of administration of the fiduciary account; (2) the person's rights with respect to objecting to the substitution; and (3) the liability of the existing fiduciary and the substitute fiduciary for their actions.
