31A-44-202. Registration.
(1) To register under this part, a person shall: pay an original registration fee established by the department in accordance with Section; and 63J-1-504 submit a registration statement, in a form approved by the department, that contains the information described in Subsection. (2)
(2) A provider's registration statement shall include: the provider disclosure described in Section; 31A-44-301 a copy of the continuing care contract that the provider will propose to a prospective facility resident; evidence that the provider's facility is located or will be located in a zone that a municipality or county has zoned for continuing care facilities; and information required by the department by rule made in accordance with. Title 63G, Chapter 3, Utah Administrative Rulemaking Act
(3) The department shall accept or deny a registration no later than 180 days after the day on which the provider applies for registration.
