80-4-202. Contents of petition.
(1) A petition for termination of parental rights shall include, to the best information or belief of the petitioner: the information required by Utah Rules of Juvenile Procedure, Rule 17; the grounds on which termination of parental rights is sought, in accordance with Section; and 80-4-301 the names and addresses of the individuals or the authorized agency to whom legal custody or guardianship of the child might be transferred.
(2) The petitioner shall attach a copy of a relinquishment or consent, if any, previously executed by the parent or parents to the petition described in Subsection. (1)
