Application; contents. (a) The application shall be on a form prescribed by the secretary of state and shall include: (i) The proposed bill to be initiated; (ii) The designation of a committee of three (3) applicants who shall represent all sponsors and subscribers in matters relating to the initiative; and (iii) A statement that the committee of applicants are qualified registered voters who signed the application with the proposed bill attached.
