Mallinckrodt Pharmaceuticals (Dublin Ireland; St. Louis MO operations) — once a major US specialty pharmaceutical company including the only significant US producer of acetaminophen API — filed for Chapter 11 bankruptcy in October 2020, having agreed to pay $1.6 billion to resolve opioid lawsuits brought by state attorneys general and municipalities. Mallinckrodt's opioid liability (from its SpecGx division's production of generic oxycodone and other Schedule II narcotics) destroyed the company's financial position. Mallinckrodt emerged from bankruptcy in June 2022 — then filed Chapter 11 again in August 2023 when its opioid settlement trust ran into additional claims. The repeated bankruptcies eliminated Mallinckrodt's capacity and management focus from non-opioid businesses including its acetaminophen API production, effectively removing the US's last significant domestic APAP API production capability. Mallinckrodt's corporate history spans 155+ years and includes some of the most notorious episodes in American pharmaceutical pricing history. Founded in 1867 in St. Louis by the German-American Mallinckrodt brothers as a fine chemicals company, Mallinckrodt became a specialty pharmaceutical manufacturer over 150 years. The company acquired Acthar Gel (repository corticotropin injection, derived from pig pituitary glands) from Questcor Pharmaceuticals in 2014 — the product whose price had already been raised from ~USD 40/vial in 2001 to over USD 36,000/vial by 2014. Under Mallinckrodt ownership, Acthar reached ~USD 40,000+ per vial — making it one of the most expensive drugs in America by per-dose cost. Senate Finance Committee and DOJ investigations into the pricing followed. Mallinckrodt's opioid manufacturing also contributed to the US opioid crisis, generating ~USD 1.7B in legal settlements that drove it into Chapter 11 bankruptcy in 2020 and again in 2023 — both times, the Raleigh APAP plant continued operating because it was operationally and legally separate from the opioid and Acthar controversies.
