Octapharma AG
Octapharma AG (Lachen, Schwyz Switzerland; private; founded 1983 by Wolfgang Marguerre who remains owner; ~€3.5B revenue) is the world's largest privately-owned plasma fractionator — and the largest plasma company not listed on a stock exchange. Octapharma produces IVIG (Octagam, Gammaplex), albumin (Albiomin), Factor VIII/IX (Nuwiq, Wilate), and clotting factors from plasma collected at its 250+ OctaPlasma collection centers in the US and Europe. Octapharma's willingness to remain private gives it long-term investment horizons that publicly-traded competitors face pressure against. Octapharma operates fractionation facilities in Vienna Austria, Springe Germany, Stockholm Sweden, Copenhagen Denmark, and Lingolsheim France. Founded by a single Austrian pharmacist, Octapharma has grown to become a major global pharmaceutical company through retained earnings rather than capital markets.

Octapharma's plasma fractionation platform simultaneously produces plasma proteins for two different patient populations with entirely different care settings: (1) outpatient/chronic disease — IVIG infused every 3-4 weeks by immunodeficient patients at home or infusion centers who take it continuously for years to prevent life-threatening infections; and (2) inpatient/surgical — fibrin sealants and fibrinogen concentrates (Tisseel-equivalent, Haemocomplettan) applied during surgery to stop bleeding at anastomosis sites, on liver and spleen wounds, and in neurosurgical procedures. The same plasma donor in a Frankfurt OctaPlasma center contributes to both the IVIG bottle in an immunodeficient child's monthly infusion AND the fibrin sealant used in a cardiac surgeon's bypass anastomosis. Octapharma's fractionation process separates different proteins from the same human plasma, with IgG going into IVIG and fibrinogen/clotting factors going into surgical sealants — two supply chains (outpatient chronic disease management and inpatient acute surgical care) from the same donor plasma.
Octapharma AG ↗Origin2023
Octapharma AG was founded in 1983 in Lachen, Schwyz (Switzerland) by Dr. Wolfgang Marguerre — an Austrian pharmacist who started the company with personal capital after identifying plasma fractionation as a growing pharmaceutical need. Marguerre's founding insight was that plasma-derived proteins would become essential medicines as the immunodeficiency, hemophilia, and critical care markets expanded. Octapharma has remained 100% privately owned by Marguerre since founding — never taking outside investors, never conducting an IPO — growing from zero to approximately €3.5B revenue through retained earnings over 40 years. This private ownership structure is unusual at Octapharma's scale: among the world's top 5 plasma fractionators (CSL, Grifols, Octapharma, Takeda PDT, Kedrion), Octapharma is the only one that is neither publicly listed nor owned by a publicly-listed parent. Marguerre's personal ownership of a €3.5B healthcare company — sustained for 40 years without capital markets — represents a capital discipline and long-term orientation that is exceptional in the pharmaceutical industry.
