PARI Medical's Altera nebulizer is the ONLY device cleared by FDA for use with Cayston (aztreonam for inhalation solution, by Gilead) for cystic fibrosis lung infection. This creates an unusual supply chain structure: Gilead's pharmaceutical drug supply chain is downstream-locked to a single German device manufacturer. If PARI Altera production were disrupted, Cayston would become unusable for the CF patients who depend on it even if the drug itself remained available — the device and the drug are a regulatory co-dependency. PARI Medical was founded in Munich in 1906 as Pari-Werk, manufacturing atomizers and inhalers before World War I — making it one of the oldest continuously operating respiratory device companies in the world. Having survived both world wars and Germany's postwar industrial reorganization, PARI relocated to Starnberg near Munich and reinvented itself as a precision medical device company in the postwar era. Its eFlow mesh nebulizer platform, developed in the 1990s-2000s, required the same precision metalworking tradition used in Bavaria's instrument-making industry — the vibrating perforated membranes with micrometer-scale apertures were a natural extension of the precision fabrication PARI had applied for a century to jet nebulizer components.
