Gives simple meanings for key words used in this part. Code set — a list of codes used to record data, like tables of terms, medical concepts, diagnosis codes, or procedure codes. Health care clearinghouse — a public or private group that turns nonstandard health data into standard data. Health care provider — includes providers named in sections 1395x(u) and 1395x(s) and anyone who gives health care services or supplies. Health information — any spoken or recorded information made or received by a provider, health plan, public health authority, employer, life insurer, school or university, or clearinghouse that is about a person’s past, present, or future health, care, or payment for care. Health plan — an individual or group plan that pays for medical care and includes many types, such as group health plans (only if they have 50 or more participants or are run by an entity other than the employer), health insurance issuers and HMOs, certain parts of the Medicare program under subchapter XVIII, the Medicaid program under subchapter XIX, Medicare supplemental policies, long-term care policies (unless the Secretary says they do not provide enough coverage), employee welfare benefit plans for employees of 2 or more employers, the military health program under title 10, the veterans health program under chapter 17 of title 38, CHAMPUS as defined in section 1072(4) of title 10, the Indian Health Service program under the Indian Health Care Improvement Act, and the Federal Employees Health Benefit Plan under chapter 89 of title 5. Individually identifiable health information — health information (including demographic data) made or received by a provider, health plan, employer, or clearinghouse that identifies a person or could reasonably be used to identify them. Standard — a data element or transaction that meets the standards and implementation specifications the Secretary adopted under sections 1320d–1 through 1320d–3. Standard setting organization — an American National Standards Institute–accredited group, including the National Council for Prescription Drug Programs, that develops needed standards. Operating rules — the business rules and instructions needed for electronic information exchange that are not set out by a standard or its implementation specifications.
