Grants must require a system of care to make and carry out an individualized plan of services for every child it serves. The family must take part in making the plan, and the child must too unless a clinician says that would be inappropriate. A team of qualified service providers must create the plan and must review and update it at least once a year. That team should include, as needed, mental health, other health, educational, social, and vocational counseling and rehabilitation providers. If the child has an Individualized Education Program (IEP) under the Individuals with Disabilities Education Act, the team must consider the IEP, make the service plan match and coordinate with the IEP, and make sure any written agreement with the school system covers these requirements. Each plan must say what services the child needs, provide the services that fit the child (including vocational counseling, rehabilitation, and transition services for children 14 and older), set goals and how to reach them, and name a person responsible for case management or confirm that the IEP will provide that case management.
