Title 42 › CHAPTER 6A — PUBLIC HEALTH SERVICE › SUBCHAPTER XV — HEALTH INFORMATION AND HEALTH PROMOTION › § 300u–13
The Secretary of Health and Human Services, through the CDC Director, must award competitive grants to state and local government agencies and community groups to run, test, and share proven community health programs. The goals are to lower chronic disease, prevent related conditions, reduce health disparities, and build better evidence about what works. At least 20 percent of the grants must go to rural and frontier areas. Eligible applicants include state or local government agencies, national networks of community groups, state or local nonprofits, and Indian tribes. Applicants must send an application with required details and show they can work with partners across the community. Grantees must give the CDC a detailed plan for policy, environment, program, and infrastructure changes to promote healthy living and cut disparities. Plans can cover things like healthier schools, places to be active and get nutritious food, programs for nutrition, exercise, smoking cessation, mental health, worksite wellness, healthier restaurant choices, and actions to reduce racial and ethnic disparities and help special populations (all ages, people with disabilities, urban, rural, and frontier). Grants may include in-kind support like staff or space. Grantees must measure outcomes such as weight, nutrition, physical activity, tobacco use, emotional well-being, BRFSS data, and other measures set by the Secretary, and file an annual report. Grantees must meet at least once a year to share lessons and create models others can copy. The CDC must train grantees, give plan feedback, and provide evaluation guidance with expert help. Grant money cannot be used to create video games or any activities that increase obesity or inactivity. Funds were authorized as needed for fiscal years 2010 through 2014.
