Title 42 › CHAPTER 35 — PROGRAMS FOR OLDER AMERICANS › SUBCHAPTER IV — ACTIVITIES FOR HEALTH, INDEPENDENCE, AND LONGEVITY › Part A — Grant Programs › § 3032c
The Assistant Secretary must give grants, after talking with the State agency, to public agencies and nonprofit groups. Grants can pay part or all of the cost to start or run model health care projects through multipurpose senior centers in rural areas that provide nutrition services under section 3030e. These projects can include home health care, adult day care, mental health services, outreach, and transportation. The goal is to meet the health care needs of medically underserved older people who live in those rural places. To get a grant, an applicant must send an application to the Assistant Secretary with required information and promises. The application must show the applicant’s experience with the planned medical services; work with graduate programs in fields like public health, nursing, social work, nutrition, gerontology, and similar areas; and coordination with critical access hospitals and rural health clinics. The project must be at a multipurpose senior center in a rural area with fewer than 5,000 people or in a county with fewer than seven people per square mile. The center must be in a State where at least 33 1/3 percent of the population lives in rural areas and at least 5 percent lives in counties with fewer than seven people per square mile, using the latest Census data. Applicants must also agree to send required evaluations and reports. The Assistant Secretary must send Congress a report that summarizes those evaluations and reports.
