Title 42 › CHAPTER 35 — PROGRAMS FOR OLDER AMERICANS › SUBCHAPTER IV — ACTIVITIES FOR HEALTH, INDEPENDENCE, AND LONGEVITY › Part A — Grant Programs › § 3032k
Makes the Assistant Secretary give competitive grants to qualified groups to create local projects that help older people stay in their homes and communities. Grants last 3 years. Eligible groups are nonprofits, community organizations, area agencies on aging, tribal organizations, local governments, or others the Assistant Secretary approves that have experience serving older adults. Groups that run certain federal housing service programs are not eligible. A "Naturally Occurring Retirement Community" is a place with a lot of older residents — either at least 40% of household heads are older, or enough older people live there to make service delivery efficient — and it is not a nursing home or assisted living facility. Applicants must show past experience and describe the project area. They must do a needs assessment that lists current services, gaps, the needs of older residents, and services that are missing. Applications must include a plan for an innovative way to coordinate and deliver services, ways to improve existing services, steps to avoid duplicating other funded programs, plans to work with public and private partners, referral procedures for long-term care, opportunities for older people to help run the project, a plan for long-term funding, and agreement to provide evaluations and reports. Grant money must be used to make sure older people can get community-based services such as case management and social work, health care management and prevention, social and recreational programs, and volunteer activities; to reach out to older residents; and to develop new, cost-effective service models (which may include mental health). Projects must coordinate with organizations funded under subchapter III, try to serve low-income communities and places with many older residents (like NORCs), and use the funds to add to, not replace, other funding. The Assistant Secretary may also fund a nonprofit to give technical help to grantees. Each year the Assistant Secretary must report to Congress on findings, best practices, and recommendations.
