You must submit two VA forms to apply
If you apply for a Specially Adaptive Housing (SAH) Assistive Technology grant, you must submit VA Form 26-0967 (a certification about debarment, suspension, ineligibility and voluntary exclusion) and VA Form 26-0967a (statements addressing six scoring criteria). Loan Guaranty personnel will use these forms to decide if you meet the requirements for an SAH Assistive Technology grant under 38 U.S.C. 2108.
