Hospitals Keep Voluntary Safety Survey
AHRQ will reinstate, without change, the Surveys on Patient Safety Culture (SOPS) Hospital Database (OMB No. 0935-0162) so U.S. hospitals can voluntarily submit SOPS Hospital Survey 2.0 data (40 items) to the database. Hospitals or their vendors that submit data receive a feedback report and an aggregated, de-identified report of other hospitals' findings to help identify strengths and areas for improvement in patient safety culture.
