Facility Licensees Must Continue Form 396
If you are a facility licensee who certifies medical fitness for operators, you must continue to complete NRC Form 396. The form is required upon application for an initial or upgrade license, every 6 years for renewal of an operator or senior operator license, and for notices of disability that occur during licensed tenure.
