The JADE Act can render certain people involved in specified Burmese organizations or activities, and their immediate family members, ineligible for U.S. visas.

Mandatory JADE Act Visa Questionnaire

If you are a Burmese national applying for a U.S. visa, you must complete form DS-5537 (the JADE Act Questionnaire) once per application and bring a signed copy to your consular interview. The form is available on Embassy Rangoon’s website or can be provided by consular staff; immigrant applicants receive the questionnaire by email and nonimmigrant applicants typically receive it through their appointment confirmation or the embassy website.
