Death Gratuity up to $100,000

If a federal or Non-Appropriated Fund Instrumentality (NAFI) employee dies from injuries incurred in connection with service with the Armed Forces in a contingency operation, eligible beneficiaries may receive a death gratuity payment of up to $100,000 under 5 U.S.C. 8102a.

Retroactive Payments for OEF/OIF Deaths

Families of federal and NAFI employees who died in the theater of Operation Enduring Freedom or Operation Iraqi Freedom on or after October 7, 2001 may be eligible for retroactive death gratuity payments of up to $100,000.

Claims and Agency Notification Forms (CA-41, CA-42)

OWCP Form CA-41 is used by named beneficiaries and possible recipients to file claims and provide information so OWCP can determine eligibility; OWCP Form CA-42 is used by agencies to notify OWCP immediately upon the death of a covered employee to initiate benefit administration.

Optional Beneficiary Designation Form (CA-40)

You (a federal employee) may use OWCP Form CA-40, an optional form, to vary the order of precedence of beneficiaries or to name alternate beneficiaries for the death gratuity if you wish to do so.
