Electronic Crisis Assistance Form

You can voluntarily complete an electronic Crisis Assistance Request Form when a foreign-country crisis is activated to tell the Department of State your exact location, plans, group size, and contact information. The Department estimates up to 120,000 respondents, the form takes about 5 minutes to complete, and the link is only activated when there is a need.

Possible Government-Coordinated Departures

If local conditions permit, the Department may use the information you provide to arrange U.S. government‑coordinated departure assistance, including chartered or non‑commercial transportation, based on the data entered in the form. Availability of such departures can vary depending on the nature of the crisis.

Eligibility Timing for LPRs and Family

You should know that U.S. citizens are always eligible to complete the form when it is activated; Lawful Permanent Residents (LPRs) and eligible family members may be included later in the crisis rather than at the initial activation. The Department will decide during the crisis whether to activate those additional respondent options.
