A temporary 10 percentage point boost to the Federal medical assistance percentage (FMAP) for Medicaid home and community-based services (HCBS). This change would target HCBS spending in fiscal years 2026 and 2027 and is capped so no State’s FMAP exceeds 95 percent. States must apply and agree to use the funding for specific workforce and service improvements.

  • Families and people who need long-term supports could get faster access and more services. States may use funds to resume HCBS for people on waiting lists and to help eligible individuals move back home, including moving costs and first month’s rent.
  • Workers who provide home care and direct support could see higher pay, paid sick and family leave, hazard or overtime pay, and more training. The law requires that rate increases be passed through to home health workers or direct support professionals.
  • States must submit an application describing planned activities, promise to spend funds by September 30, 2029, and assure the federal funds will supplement state HCBS spending. The Department of Health and Human Services must certify applications, require reporting, and fund an external evaluation of access and quality by December 31, 2029.
