State employee plans and self-insured city, village, town, and school plans must follow the breast imaging coverage rules. Defined network and preferred provider plans must follow the mammogram rules, too. Plans may still apply their normal network requirements, like using in-network providers.
Wisconsin Medicaid covers the breast cancer screening and diagnostic services listed in state law. Eligible members can get these tests through Medicaid. This improves access to needed imaging for people on Medicaid.
Health plans must cover diagnostic breast exams. Plans must also cover extra screening if you are high risk under the latest NCCN guidelines or have heterogeneously or extremely dense breasts under BI-RADS. Covered tests include MRI, ultrasound, tomosynthesis, contrast-enhanced mammography, diagnostic mammography, and other NCCN-backed technology. You pay no copay or coinsurance for these exams. If your plan is an HSA-qualified high-deductible plan, you may have to meet the federal minimum deductible first; preventive care still applies before the deductible. Coverage applies even without symptoms, and plans can only use limits that match other radiology.
The law takes effect on the first day of the fourth month after publication. If your policy conflicts with the law, the new rules start for policy or plan years beginning January 1 of the year after the law’s start date. For plans under a union contract, the new rules start on the law’s effective date or when the contract is new, extended, changed, or renewed, whichever is later.
