The Election Assistance Commission must work with state chief election officials to write rules, create a mail voter registration form for federal elections, send Congress a report by June 30 of each odd-numbered year that reviews the last 2-year period and offers improvement ideas, and give states information about their duties under the law. The mail form can only ask for the data needed to check eligibility and run registration (including the applicant’s signature and past registration info). It must list eligibility rules (including citizenship), include an attestation that the applicant meets them and a signature under penalty of perjury, not require notarization, include two other pieces of information the law requires elsewhere, and state that a choice not to register and the office where someone registers are kept confidential and used only for voter registration.
