If you or your designated proxy are interviewed about a reported case of cryptosporidiosis, you may be asked to complete the CryptoNet Case Report Form by telephone. The form is voluntary, is estimated at 15 minutes per response, and CDC expects up to 500 responses per year (about 125 total annual burden hours); there are no costs other than your time.
