Grantees Must Upload De-Identified EHRs
If your clinic received a SAMHSA CCBHC-Expansion grant, SAMHSA will ask you to upload de-identified client-level electronic health record (EHR) data during Quarter 4, 2025 and during Quarter 3, 2026. The upload must include client demographics and specific assessment tools (Patient Health Questionnaire, Columbia-Suicide Severity Rating Scale, GAD-7, AUDIT, and DAST). SAMHSA estimates 298 respondents will each submit 2 uploads of about 8 hours each (16 hours per respondent), for a total burden of 4,768 hours, an average hourly wage of $59.07, and total respondent cost of $281,645.76.
