Agency Extends Longstanding Injury Treatment Form

Published Date: 6/9/2026

Notice

Summary

The Department of Labor wants to keep using a form called LS-1, which helps workers ask for medical exams or treatment related to work injuries. They’re asking for your thoughts by August 10, 2026, to make sure the form is easy to use and doesn’t waste time or money. This affects workers filing claims and the agencies handling them, with no new costs expected—just a smooth extension of the current process.

LS‑1 Keeps Authorizing Medical Care

If you are a worker injured in maritime employment covered by the Longshore and Harbor Workers' Compensation Act, Form LS-1 will continue to be used to request and authorize medical examination and treatment and to let the treating physician report findings and expected recovery. The form is collected under 33 U.S.C. 907 and 20 CFR 702.419 and OWCP is proposing an extension of its current use without change.

Paperwork Time and Cost to Respondents

The OWCP estimates 136 respondents will fill Form LS-1 on occasion, with one response each, averaging 65 minutes per response. OWCP reports annual burden of 147.33 hours and an annual respondent cost of $3,835.09 associated with this collection.
