If you want OWCP to reimburse travel for covered medical care under FECA, BLBA, or EEOICPA, you must submit the OWCP Medical Travel Refund Request—Mileage (OWCP-957A) or the Medical Travel Refund Request—Expenses (OWCP-957B). The collection (OMB Control No. 1240-0037) covers 564,388 respondents and 1,128,776 responses with 56,438.80 annual burden hours and $1,896,343.68 in annual respondent cost; for BLBA claims the physician’s signature is required on the form.
