Centralized Case-Level Data Collection

The Department will collect and maintain a centralized case-level database with information about sexual assaults, including the nature of the assault, victim, alleged offender, investigative details, and case outcomes as directed by law. The collection covers the listed Defense forms (DD Form 2965, 2910, 2910-1, DD2910-2, DD2910-8) under OMB Control Number 0704-0482.

Return of Forensic Examination Property

Under Public Law 116-92 (FY2020 NDAA), a victim who files a restricted sexual assault report may request at any time the return of their personal property obtained as part of the sexual assault forensic examination. The Department will prescribe procedures to enable such requests.

Time Burden to Complete Forms

If you fill out a Defense sexual assault report form, the Department estimates an average burden of 2.974 hours per response. The agency estimates 8,245 respondents and a total annual burden of 24,521 hours for these forms.

Retaliation Claims Included in Annual Report

The Department will include information on each claim of retaliation tied to a sexual assault report in the Annual Report on Sexual Assault in the Military, including narrative descriptions, complainant and alleged retaliator details, and investigation summaries, as required by Public Law 114-328 (FY2017 NDAA).
