Certification for Survivor Benefits

You (or the child’s guardian) must provide form RI 25-41 to certify that an unmarried child between the ages of 18 and 22 is a full-time student so the Office of Personnel Management (OPM) can decide survivor benefit eligibility. OPM says it uses RI 25-41 to determine whether unmarried dependent children (between 18 and 22 years of age) may receive survivor benefits from deceased Federal employees or annuitants.
