The Department of Defense plans to continue collecting DD Form 370 reference responses from individuals. The notice says there will be 50,000 respondents who each provide one response that takes about 10 minutes on average, totaling 8,333 annual burden hours; responding is voluntary.

Form used for waiver requests for applicants with offenses

DD Form 370 is used to obtain personal reference data to request a waiver for a military applicant who has committed a civil or criminal offense and would otherwise be disqualified from entry into the Armed Forces. The form collects evaluations of the applicant's character, work habits, and attitudes from a person of authority or standing in the community.
