New CHAF form for VISTA applicants

If you are a VISTA sponsor or a candidate for VISTA service, a new AmeriCorps VISTA Criminal History Acknowledgement Form (CHAF) must be completed in certain cases — for example, when a candidate fails to report criminal history or is noted to have certain violent or serious offenses. The agency estimates 240 total annual responses and 120 total annual burden hours, and the public comment deadline is November 13, 2026 (OMB Control Number 3045-NEW).

Sponsors give formal fit recommendations

VISTA sponsors or local site supervisors must complete Part C of the CHAF to recommend whether to accept or reject a candidate based on disclosed criminal history; they must explain why the candidate is or is not a good fit and, if accepting, describe measures and supervision they will provide. AmeriCorps VISTA retains final eligibility and suitability decisions under applicable law.

Ways for candidates to dispute background findings

Candidates receive their background check results directly from the contractor 'Checkr' and may dispute inaccurate or incomplete findings through that system; the CHAF also provides a place for candidates to address discrepancies, document clarifications, and elevate issues to VISTA if necessary. Part A of the CHAF asks candidates to report offense details (offense, date, final court judgment, legal status) and Part D requires a personal narrative for violent offenses.
