Private plans: no-cost imaging
If enacted, group and individual health plans would have to cover and not charge cost-sharing for the listed breast imaging for plan years beginning on or after January 1, 2026. This applies to ERISA plans, individual-market plans, and applicable grandfathered plans for those plan years. Covered tests and allowable frequency would follow ACR BI-RADS, ACR Appropriateness Criteria, and NCCN guidance. Coverage would apply to people at increased risk, people with dense breasts, and when a clinician orders imaging under the guidelines.
Medicare breast imaging coverage
If enacted, Medicare Part A and Part B would cover the listed breast imaging for services furnished on or after January 1, 2026. Covered tests would include mammograms (2D and 3D), breast ultrasound, breast MRI, molecular breast imaging, and other NCCN-recommended technologies. Coverage would apply to people at increased risk, people with heterogeneously or extremely dense breasts, and when a clinician determines imaging is needed under the listed criteria. Medicare Advantage plans would also have to cover these services without patient cost-sharing and follow NCCN frequency rules.
TRICARE: no-cost breast imaging
If enacted, TRICARE would cover the listed breast imaging for eligible members and former members starting January 1, 2026. TRICARE Prime and TRICARE Select would not require copayments for these services when furnished at NCCN-recommended frequency. Eligibility would follow ACR and NCCN criteria, BI-RADS density categories, or a clinician's determination using the applicable guidelines.
VA: no-cost breast imaging
If enacted, the VA would provide the listed breast imaging to eligible veterans starting January 1, 2026. The VA would not charge copayments for these services even if other VA copayment rules apply. Coverage would apply to veterans at increased risk, veterans with dense breasts, or when a clinician determines imaging is needed. The VA could provide this care regardless of a veteran's enrollment status in the annual VA patient enrollment system.
Medicaid: no-cost breast imaging
If enacted, Medicaid would cover screening and diagnostic breast imaging starting January 1, 2026. Covered tests would include 2D and 3D mammograms, breast ultrasound, breast MRI, and other NCCN-recommended technologies. Medicaid would not charge copays for these services when done no more often than NCCN guidance. States could delay implementing the rule if HHS finds state legislation is needed, which may postpone access for some enrollees.
