SVC Enrollment Requires Applicant Info

If you apply to the Treasury Stored Value Card (SVC) program, you must provide information on FS Form 2887 to enroll. Disclosure is voluntary, but failing to provide the requested information may significantly delay or prevent your participation; the agency estimates 102,030 respondents, about 10 minutes per applicant, and 17,001 total annual burden hours.

Contractors Must Authorize Bank Debits

If you participate as a contractor in the Treasury SVC program, you must use FS Form 2889 and FS Form 5752 to provide information and to authorize the Treasury to initiate debit and credit entries to your bank or credit union accounts. FS Form 2889 is estimated at 10 minutes and FS Form 5752 at 1 minute; these authorizations also facilitate collection of any delinquent amounts.
