New Privacy Act Request & Consent Forms

If you ask the NCUA for records about yourself or allow the NCUA to share those records, the agency will use two new forms: "Request for Individual Access to Records Protected Under the Privacy Act" to verify your identity and find the right system(s) of records, and "Consent for Disclosure of Records Protected Under the Privacy Act" to get your permission and the contact information for any third party. The collection is being submitted to OMB as OMB Number 3133-NEW and the agency estimates the total annual burden for respondents is 16 hours.

Public Comment Deadline and Record Notice

You can send written comments about these information collections to the NCUA by March 30, 2026. All comments submitted will be summarized, included in the request to OMB, and will become a matter of public record.
