The SBA plans to request OMB approval to extend or revise information collection OMB Control Number 3245-0172 using SBA Form 1405 (Stockholder's Confirmation) and Form 1405A (Ownership Confirmation). These forms are used as part of statutory SBIC examinations, which must occur at least biennially under 15 U.S.C. 687b(c); SBA estimates 690 annual respondents and 690 annual hours of burden.
