OGIS Consent Form for FOIA Help

If you ask the FOIA Ombuds (OGIS) for help with a Freedom of Information Act (FOIA) request or appeal, you may be asked to sign NA Form 10003 to let agencies share records about your request. NA Form 10003 is available as a fillable PDF with an electronic signature option that you can email to OGIS now, and OGIS says it hopes to add online submission in a future case management system which could speed how OGIS helps solve FOIA disputes.

NARA estimates 30 respondents will use the consent form each year, at about two minutes per response, for a total estimated annual burden of 1 hour. The notice explicitly states this collection "does not have a significant impact on small businesses."
