Dentists set noncovered fees; fair rates for covered care
Insurers and HMOs cannot make network dentists use plan-set prices for services the plan does not cover. For covered dental services, plans must set fees in good faith, and the fees may not be nominal. A "covered dental service" includes care with a deductible, copay, or coinsurance. It also includes services limited by waiting periods, annual or lifetime caps, frequency limits, or an alternative benefit. These rules apply statewide to individual plans, group plans, and HMOs.
