Insurers gain flexibility on community paramedicine
For covered community paramedicine care, insurers do not have to pay ambulance providers the otherwise required ambulance reimbursement rates. Insurers may also require approval before care begins. The law does not itself require patients to pay or set their out-of-pocket costs.
New licensing safeguards for community paramedicine
Ambulance services and nontransport emergency medical services need state licenses to provide community paramedicine, replacing the former authorization. Clinicians need a board license, a separate emergency medical services license, instructor-led training, and proof of trustworthiness and skill. The exemption covers only licensed services providing community paramedicine alone, through single visits without an ongoing general home-care relationship. The Maine Emergency Medical Services Board must require background checks and initial and ongoing training for all staff on mandatory reporting duties. Board rules must set staff, organization, treatment and business record standards, and require coordination with home health agencies.
