Older hospital cost reports settled in 240 days

The law applies its timelines to cost reports filed before the effective date. DPHHS must settle all of those reports within 240 days after the law takes effect. This speeds up final payments or paybacks on older hospital reports.

Faster Medicaid settlements for critical access hospitals

Critical access hospitals file annual Medicaid cost reports. DPHHS makes a tentative adjustment when the Medicare contractor gets your report and accepts your costs unless there are obvious errors. DPHHS must make an interim settlement and any payment by the earlier of 240 days after the contractor gets your report or 30 days after DPHHS gets the contractor’s interim settlement. After the contractor’s final desk review or audit, DPHHS makes a final adjustment; if you are owed money, it pays within 90 days of notice. If an overpayment is found at either step, you must repay or set a plan within 60 days of notice. The law uses the federal definition of “Medicare administrative contractor,” including intermediaries.

Funds to process Medicaid hospital settlements

The law gives DPHHS $33,900 from the state Medicaid hospital account and $33,900 in federal funds for the year starting July 1, 2025. It also gives $7,500 from each fund for the year starting July 1, 2026. The legislature intends $15,000 from each fund over the biennium to be part of the ongoing base. This money supports processing and paying the hospital cost settlements.
